Each section of the form must be completed prior to issuing the permit.
Section 1 is the Student Information (including Social Security #),
Section 2 must be completed by the parent/guardian.
Section 3 must be completed by the employer/supervisor.
Once the form is completed please drop it off at the GHHS Guidance Office or email it to Alondra Tapia at atapia6123@portervilleschools.org during the school year, and during the summer, Maricela Felix at maricelafelix@portervilleschools.org. Please allow for 1-2 days for processing. Paper Copies of the Work Permit Forms are also available at GHHS Admin Office or GHHS Guidance Office.
