What the form will ask for:

• The reason for the absence

• Anticipated return date

• Homework pick-up requests (optional)

Important Note on Excuse Slips: Submitting this form counts as your official excuse note! However, if your child is out sick for more than three consecutive days, a physical doctor's note is still required when they return to school.
