IMPORTANT Dates and Information
OhioHealth Sports Medicine and OhioHealth Physician Group Marion are pleased to announce that they will be offering NO CHARGE Sports Physicals again this summer. The process will be similar to the previous two years.
REMINDERS:
Pre-participation physicals are best performed with the student-athlete’s primary care physician.
A parent/guardian must accompany the student-athlete to the sports physical appointment.
Masks will be required to be worn at all times per OhioHealth COVID-19 guidelines.
There will be no charge to the student-athlete for the sports physical

Student athletes will be required to complete a physical before the start of their athletic season. There is a specific physical form required by the Ohio High School Athletic Association, which must be used. There are several items on the form that need to be filled out by the athlete or parent, including a required parent signature.
These forms are available through the OhioHealth sign-Up process OR by clicking the link below as well as in both the middle school and high school offices. Forms and information can also be found on the Pleasant Local Schools website (www.pleasantlocalschools.org) and the Ohio High School Athletic Association’s website (www.ohsaa.org).

PAY TO PARTICIPATE
HS Extra-Curricular Activities (Athletics, Music & Drama) are $75.00 per year/per child.
MS Extra-Curricular Activities are $50.00 per year/per child.
There is a $150.00 district wide family cap for participation in all extracurricular activities.
Pay to participate can be paid when your student attends and pays for drug testing.
