The files at the bottom of the page are the most commonly requested athletics forms. You can pick these forms up at the school, the Main Office, or download and print them from the website. Your student can also request these forms from their physical education teacher and/or coach.

The E.B. Aycock Athletic Department must have the following forms on file in order for your child to try out and participate in sports at the school.  (You can print any of these files from below)

You must use the NORTH CAROLINA HIGH SCHOOL ATHLETIC ASSOCIATION form!

Make sure you and your child sign EVERY FORM and INITIAL the concussion form on the Athletic Participation Forms.
