Health and Immunization Requirements
All Cleveland County School students are required to submit a record of immunizations required in accordance with NC law in the first 30 days of school attendance. There are immunizations required for kindergarteners, 7th graders, and 12th graders
The North Carolina General Statutes (G.S. 130A-152(a)) requires immunizations for every child present in this state. Every parent, guardian or person in loco parentis is responsible for ensuring that their child(ren) receives required immunizations. Immunization records are reviewed when a child enters a school.
NC Health Assessment Form
In addition to required immunizations, students entering a NC public school for the first time are required to submit a NC Health Assessment, done any time within the 12 months before the first day of school attendance (Please see the FORMS section). The Health Assessment forms are made available at Kindergarten Orientation every spring, at your child’s school, at your health care provider’s office, or the Cleveland County Health Department Child Health Clinic. Please be aware that appointments fill up rapidly and may be difficult to obtain after school starts. Help your child get a good start by making his/her appointment as soon as possible (within 12 months before the first day of school).
Exceptions to these requirements are made only for students for Bonafide religious reasons in the case of immunizations, or for medical reasons approved by y physician.

If the requirements are not met, State Law requires principals to exclude/suspend students from school until the proper information is presented to the school for filing in the students’ permanent health record
