Parents/guardians must send medications to school in a sealed, original container labeled with the following information.
- The student’s name and grade
- The name of the medication
- The amount of the medication to be taken
- The time the child is to take the medication
- Authorized school staff must administer ALL medication. Students are responsible for going to the office at the appropriate time to receive medication. Parents are responsible for the following: providing a signed medication form, bringing medications to the school, and picking up any leftover medications at the end of the school year. If your child attends an overnight field trip, the medicine must be brought to the school one week in advance of the trip, and a medication form must be signed for the trip.
