The Liability Waiver must be completed for each sport a student is participating in.  The Physical Form must be completed prior to the first game.  National Sports ID must be completed by the deadline mentioned in the welcome email parents receive once a student athlete is placed on a team.  Please make sure to read your welcome emails carefully, as they have important requirements and deadlines for those requirements.
