Applying to Colleges

As seniors begin applying to colleges and scholarships, they may be asked to provide letters of recommendation. Each time your senior asks someone to write a letter of recommendation, they should complete this form and provide it to the recommender at least TWO WEEKS before the recommendation is due.

The information provided on the form will help the recommender write a more detailed and personalized letter on your senior’s behalf.

Important: To complete the form, students will need to make their own copy of the Google Doc. Click File → Make a copy. Once a copy has been made, the document can be edited and completed.
