Good news! Viewing and downloading your ORF is now simpler than ever. Just follow these easy steps.
For Onsite Transaction:
1. Get a document request form at the Registrar's Office
2. Fill out the form properly, completely, and legibly then return it to the Registrar.
3. Our Registrar In-Charge will check the accomplished form and return it with the corresponding amount to be paid.
4. Proceed to the teller for the payment.
5. Go back to the Registrar and submit the request form together with the payment receipt.
6. Registrar In-Charge will then provide a "Claiming Stub" as to when will be the claiming date.
Note: For Representatives, provide the following:
Signed Authorization Letter
The ID of the Representative
The ID of the Requestor (Student/Alumni)

Note:
A passing grade in NSTP 1 & 2 (CWTS) is required.
For ROTC completers, please proceed to the Naval Office for your request. No fee will be collected.

2. After filling up this form, the student should get in touch with their faculty handler to complete any missed requirements/exams.
3. The Program Head / Dean shall entertain completion for faculty members who are not anymore connected with the University.
4. A student who gets an incomplete grade has two consecutive terms to complete it.
5. The Program Head / Dean will approve the granting of the completed grade.
6. Failure to get a passing grade for an incomplete grade after the elapsed of two terms would mean that the student will get a No Credit (NC) mark in the subject concerned.
7. For FIRST SEMESTER 2023-2024, to process your completion request, a corresponding completion fee shall be applied:
for SGPS / Law students, 150 pesos per subject; and
for Tertiary students, 100 pesos per subject.

Obtain the Shifting Form from the Registrar's Office. Make sure to complete all required information accurately.
Present the filled-out Shifting Form for approval to the current Dean or approving officer of the department to which he/she belongs for approval.
Submit the approved Shifting Form to the Dean approving officer of the desired department for signature.
Once you have completed the necessary approvals and signatures, bring the completed form to the Registrar's Office.
After receiving confirmation from the Registrar, proceed to the desired Dept for proper evaluation.
Note: Shifting courses is typically done during the enrollment period for the upcoming semester or academic term.
Start by getting the Shifting Form from the Registrar's Office and fill it out carefully with all the required details.
Take the completed form to your college dean for approval.
Visit the teller to complete the payment for the dropping process.
Submit the approved form to the Registrar's Office for encoding.
Don’t forget to get your student copy for your records before leaving the Registrar's Office.
Note: Dropping courses is typically done during the enrollment period, but once enrolled in subjects, a fee of 100 pesos per subject will be charged. It is not allowed to be processed after you are officially enrolled.
Get a request form to the Office of the Registrar then fill out the requested information.
Proceed to the teller and make a payment of 50 pesos.
Wait for the release schedule in claiming of the requested GMC at the CSDL Office
Note: All Good Moral Requests are released every Wednesday of the week.
To qualify for the Dean's List, a student should meet the following criteria:
1. Academic load must be enrolled in the maximum units as prescribed in the curriculum during the semester
(e.g. must not drop any subject prescribed in the curriculum);
2. No grade lower than 2.00 in all subjects, including NSTP and PE;
3. No INCOMPLETE grade in all subjects;
4. Must have a passing grade in SSP subject/s (if any); and
5. Must not have been given any disciplinary actions.
Note: A Certificate of Recognition will be awarded to the DL awardee during our Recognition Program. Inclusion in the List of Applicants DOES NOT AUTOMATICALLY mean inclusion in the Dean's List.
