Request Letters (Template)
Click the corresponding link below and accomplish the tutorial letter/s needed for your request/s.
NOTE: Use your PHINMAEd Email account to access the files!
Click the link above for the template letter to OPEN an off-semester subject. When the Google docs has loaded, click file in the upper left corner and choose download/make a copy.
Please take note:
All student requesting to OPEN an off-semester subject MUST first seek the advice from the program head/adviser.
Only the representative will send the letter of request.
You are advised to gather at least 26 students to enroll in a regular course offering. If the number was not met, kindly ask the program head for his/her advice.
All students requesting to open MUST make sure they ALL finished the prerequisite subject/s if there is/are.
All students MUST attach their duly accomplished study plan in the email message to be sent to the program head.

Student requesting for tutorial MUST first seek the advice from the program head/adviser.
Secure proof of agreement from your tutorial instructor who will handle the course.
Make sure to FINISH all prerequisite/s required for the tutorial subject being requested.
Attach your duly accomplished study plan in the email message to be sent to the program head.
Click the link above for the template letter to request for an OVERLOAD. When the Google docs has loaded, click file in the upper left corner and choose download/make a copy.
Please take note:
Student requesting for an overload MUST first seek the advice from the program head/adviser.
Overload can only be applied once during the whole duration of your stay in the university.
Overload request can be applied during your penultimate semester in your study plan.
A maximum of 6 units can be applied for an overload.
Make sure to FINISH all prerequisite/s required for the overload subject/s being requested.
Attach your duly accomplished study plan in the email message to be sent to the program head.
