Hi Everyone,

I think we all showed great resolve during our temporary relocation at the Austin Meehan site, and I know we are all excited to be back at our newly renovated Pollock School!!!

The medical paperwork requirements for the 23-24 remains the same as in past years. 

• All students entering the School District of Philadelphia school system for the first time must have a complete immunization record, current physical examination, and dental examination.

• Students entering the 6th grade need to have a physical examination.

• Students entering the 3rd and 7th grades need to have a dental examination.

• Students entering 7th grade must also have an updated TDAP (tetanus, diphtheria, pertussis) vaccine and a meningoccal conjugate (meningitis) vaccine if they have not already received them. 

The above examinations and vaccines are MANDATED by Pennsylvania state law. The forms for these exams and more can be found at the bottom of this page. 

Also, if you do not have a medical doctor or dentist at  this time , these exams can be done at an area health center. The nearest health center to Pollock school is :
