ALL children are required by Pennsylvania State Law to be compliant and up to date with immunizations to attend school (both in person and online)
City health centers are operated by the Department of Public Health. They accept Medicare, Medicaid, HMO plans, and most other insurance options. If you are uninsured, the centers will charge a small fee based on family size and income for some health services. There is no fee for administration of mandated immunizations for school children. Must be filled out every year by parents or guardians
Gathers information about a child’s health history. This history can be vital in case of emergency or should the need for any type of medical treatment arise while a child is in school.
Allows the school nurse to administer Acetaminophen or Ibuprofen, or if needed Albuterol inhaler or EpiPen as prescribed by the School District’s medical Officer/pediatrician
Nurses cannot administer these medications without parent permission
ELECTRONIC FORMS COMES DIRECTLY TO THE NURSE
--If student requires daily medication in school to be administered by the school nurse, this form must be completed by Doctor, signed by parent/guardian and returned to school nurse with properly labeled medication(asthma inhalers,insulin, etc.)..No medication is allowed in school without this completed form
--Report of Physical Exam Form -REQUIRED on original entry to school (K or 1st)
--Dental Forms REQUIRED on original entry to school (K or 1st) and again at 3rd grade
--Submit an immunization exemption form if your student is exempt from any immunizations for a medical, religious or philosophical/ strong moral reason .Please note that your child will be excluded from school if a disease that they were exempted from breaks out (exclusion time varies depending on disease)
***ALL students are required by Pennsylvania law to be updated on ALL immunizations to attend school***
