Why it's important
Community Health Needs Assessment
Q: What is a community health need assessment?
A: According to the Center for Disease Control (CDC) a community health needs assessment (CHNA) is "a comprehensive picture of a community's current health status, factors contributing to higher health risks or poorer health outcomes, and community resources available to improve health."
Q: What happens once the community health needs assessment is completed?
A: A community health improvement plan (CHIP) is developed; noted by the CDC, the CHIP "is a long-term, systematic effort to address public health problems based on the results of community health assessment activities and the community health improvement process. A plan is typically updated every three to five years."

The state of Wisconsin has five priority areas in the Wisconsin State Health Improvement Plan (SHIP) 2023-2027.
Social and community conditions, including:
a. Economic well-being
b. Supportive systems of care
c. Healthy housing
Physical, mental, and systemic safety
Person and community-centered health care
Social connectedness and belonging
Mental and emotional health and well-being
Washburn County has three main focus areas in their implementation plan:
Healthy Living
Mental Health
Substance Misuse
The Implementation Plan (Strategy 3) for Mental Health states:
Education and encouragement regarding healthy habits, positive mental health strategies, and stigma reduction.
The Implementation Plan (Strategy 2) for Substance Misuse states:
Promote and/or participate in drug and alcohol-free community events.
Sawyer County 3 of the top 4 needs include:
Drug Use
Alcohol Use
Mental Health
Sawyer County’s Health Need Assessment states: “23.7% feel they or someone in their household was treated unfairly in their community in the past year.”
Barron County's top 3 listed needs to address include:
Alcohol and Substance Use
Behavioral Health
Healthy Equity
