Below you will find information on completing all the needed Athletic Participation paperwork. All PfISD & UIL forms must be completed for you/your child to participate in athletics. You must also have a Pre-participation physical exam by a medical physician and the medical history paperwork turned into Pflugerville High School Athletic Trainers. Once you have completed the online forms, Medical history, Pre-Participation Physical Exam (by a physician) and turned it into the Athletic Trainers, then you/your child will be eligible to participate in athletics (this includes practices during, before, and after school; as well as off-season workouts).
