Important Forms to be signed & returned:

• BEFORE a child is allowed to use any technology in school, the “Technology Responsible Use Form” (TRUF) must be completed, signed, and returned to the teacher.

• BEFORE a child can participate in any online sessions with their teacher, they must complete theAudio/Video Release Form. If a parent/guardian would like to only allow their child’s audio, photo, or video to be used in online sessions, yearbook, class photos, and for classroom use only, a modified version of the form can be signed.
