Permission slips will be available starting March 16th and will be distributed during breakout rooms that day.
All permission slips must be turned in at the time of purchase (the original with carbon copy).
No fax or emailed permission slips will be accepted.
Tickets on sale March 18th - May 1st
$100 per ticket
cash, money order, certified check
(made out to Pennsauken High School)
2 ticket max per senior student
(senior student + guest)
All guest paperwork must be turned in at the time of purchase.
You must have the following items ready when you purchase your ticket:
This signed contract by you and your parent/guardian (even if you are 18 years or older)
Any guest paper work completed (with outside guest photo I.D. submitted if applicable)
$100 cash, certified check, or money order made out to Pennsauken High School
TICKET POLICY:
All tickets are numbered and sold to specific individuals with a signed contract.
Tickets may not be given to, sold to or exchanged with other students.
Each student is allowed to purchase a maximum of two tickets; one for him/herself and one for a guest
Guests are permitted into the event in the company of the Pennsauken High School student and must leave when the student leaves.
The PHS senior is responsible for purchasing guest tickets. Outside guests and other grade level students are not permitted to purchase for this event.
PHS students are responsible for their guest at all times and may be disciplined for the actions of their guest. All PHS rules apply to the conduct of guests.
The purchaser of the tickets may change “guest” if:
he/she informs the other person no longer attending
he/she turns in a signed contract from the new guest at least one week prior to Prom.
NO REFUNDS WILL BE ISSUED.. This includes but is not limited to: students who do not show up for any reason (including suspension from school) or are not allowed to enter due to improper behavior, dress code violation, or use/possession of alcohol, tobacco or drugs, or became ineligible due to violation of code of conduct after ticket purchase.
Pennsauken High School administration reserves the right to refuse permission to any person to attend any event sponsored by the school.
No one leaving the Prom will be readmitted. No admittance will be granted after 7:00 PM.
Student tickets are attached to their student I.D. as provided by the school. Students must physically present their school issued I.D. to enter the event. Students who have lost their I.D. need to purchase a new one in the PHS media center. Genesis accounts will not be a feasible form of I.D. for entry to the dance.
This is a formal affair; students and their guests should dress accordingly. (Attire that is overly revealing, low cut, excessively sheer or provocative is not appropriate for any school activity. In addition, attire that does not meet the dress standards may be deemed unacceptable for admittance.)
Guest Ticket Information:
Guests must be minimally in grade 9.
Guests age may not exceed 20 at the time of the event.
Guests are expected to follow the School District’s Code of Conduct. Guest need to be aware of and understand the terms expressed in the Dance Information Guideline and agree to follow those terms. If at any time during the event the guest is deemed non compliant to the guidelines, s/he understands that s/he may be reported to administration and ejected from the event. Guests take on full responsibility for their actions and will be prosecuted for any unlawful activities.
Outside Guests who have already graduated/are not enrolled in school:
The top half (with signatures) of the outside guest permission slip still needs to be completed & submitted.
Instead of the bottom half being completed since they are not enrolled in the school any longer please write in the following information:
Name of last school attended
Address of last school attended
Phone # of last student attended
All outside guests need to have a photo ID on file that will be presented at the point of entry for the dance.
