PTHS Transition Resources

Assistance for the Road Ahead

Welcome!

Before we know it, our students will come to the end of their high school careers. This transitional period offers many opportunities and challenges as students prepare to leave high school and make plans for the future. While students are with us in school, case managers, guidance counselors, teachers, and other school staff are wonderful resources and provide amazing support. Because these supports cannot remain forever, our goal is to provide a seamless transition to new supports. In an effort to assist with this transition from high school to adult life, we have organized this webpage to include links to resources and agencies essential to the transition process. We hope this page provides you with information essential to preparing for a successful post-high school transition.
