Print page 14-18 of this document and have a licensed practitioner fill it in prior to the student returning to play.

If you are a student transferring to Humberview from another school, a transfer form must be filled out prior to the sport season you want to participate in. Please see Mr. Stoykoff in the PE office for assistance.
