Medication Authorization must be done on a YEARLY basis- the form is good from the first day of school through the final day
-ALL medications taken in school must be logged in through the health office. This includes over-the-counter medications such as Ibuprofen, Tylenol, and even things such as cough drops and nasal spray.
-This specific form is for any/all medications OTHER THAN anything that is included on a medical action plan (so not for inhalers or epinephrine auto-injectors)
-All medications are to be picked up at the conclusion of the school year as per Illinois law, and cannot be kept/stored past the final day. If any medications remain after the last day, they will be disposed of appropriately.
-Please note that this form has TWO sides; one must be filled out/signed by the appropriate medical professional, and the other side MUST be signed by the parent/guardian giving permission for the medicine to be administered. NO medication will be given without BOTH sides completed as indicated.
