Please read all directions before completing this form!

• For all PA Wing stations, simply list the call sign number.

• For out of wing stations, list the FULL callsign. Do NOT abbreviate. 

• Separate each station that checks-in with a comma (ie: 265, 1, 41, White Peak 123). 

• Submit this log after the completion of each daily or weekly radio net. Online submission should be made within 24 hours of the net.
