ELIGIBILITY REQUIREMENTS

BEFORE YOU CAN PRACTICE

All athletes trying out must have a physical form that has been turned in and approved by the South Houston Sports Medicine staff. 

All athletes must have completed the PISD online Rank One forms.

PHYSICAL INFORMATION

All athletes MUST have a physical on file with the Sports Medicine staff South Houston High School prior to being able to particiapte in any workout, practice or game. An athletic physical need to be completed on the 2025-2026 PISD physical form. This form is posted on the PISD website. The link to the PISD website athletic page is HERE. Rank one ​forms can be found HERE and will also be available from trainers and coaches at each campus.

RANK ONE INSTRUCTIONS

RANKONE CRASHED LAST YEAR ON THE FIRST DAY OF FOOTBALL. DO NOT WAIT UNTIL THE LAST MINUTE!!

1) To begin, click on yellow highlighted area that says “Parent/Student Fill Out Electronic Forms.

2) Click on Texas, then on Pasadena ISD and then on “Click Here” to proceed to Online forms.

3) Click on “Continue as Guest” 

In this web link, look under tab called electronic participation forms and complete player application and emergency consent sections. Every box must have something in it, so if no home phone for example, type in N/A or none.  When entering athlete information, type ID# in with “zero” included in ID#.  If parent still cannot complete online part, they need to see athletic trainers. 

After all information is entered, hit submit.  Athletes should see a page that says something along lines of information went submitted.  Both the player application and the emergency consent parts must be completed along with the completed physical in order for athletes to be cleared for participation.
