Please reserve only ONE schedule. Any duplicate reservations will be invalidated.

Be sure to report to your respective windows on your date of reservation and bring the following requirements:
  a) 1 pc. 2x2 latest picture in business attire with white background
  b) 1 pc. long size brown envelope
