Print

If you would prefer to fill out the form on paper, then you can print out the online form. Follow the steps below to print out the online form.

1. Go to the online form, which is in the Order Form page on this website, or Thad's Sticker Shop Order Form (google.com) 

2. If you are using a device with a keyboard, click CTRL + P to print. Unfortunately, you can't print from most mobile devices, so you will have to fill it out online.
