MSHSAA PRE-PARTICIPATION PHYSICAL EVALUATION
Physicals are valid for two years from the date on which exam was received. If your child’s physical is already on file, you do not need a new one, but please email a coach to verify we have a copy.
Note: Some doctors will only sign a physical for one year. If the doctor has written "For one year only" on last year's form, your athlete will need a new physical.

This document must be completed by EVERYONE, even if your physical is still on file.
Concussion Baseline Testing (SWAY) -
Required for all 2023/2024 7th and 8th grade athletes. (Will take place 8/29 during Tutorial in the morning)
Both MSHSAA forms must be complete before runner can attend practice.
An Off-Campus Running Form and Runner’s Contract signed by runner and parent/guardian. Please print and sign both pages.
One-time sports fee of $25, good for the entire school year. **This needs to be paid on Infinite Campus.
We will have a family meeting on August 15th from 5:00 to 5:30 PM in the Media Center at Walden to review expectations and events of the season.
Practice is Monday through Friday from 2:30 until 3:45PM. Practice will begin August 21st and go until our last meet at the end of September. We may have some optional Saturday practices depending on availability of coaches.
Runners must have 14 practices to participate in meets.
