We want to learn more about your experiences during distance learning. Your school and teachers will use this information to better support you this spring.

Students in grades 3-12: Please take your survey between Monday, May 4 and Wednesday, May 13.

Here's how to complete your survey:

STEP 1

(You can use any device that connects to the internet!)

STEP 2

Enter your student ID number (lunch number) in the access code box.

If you don't remember your ID number, please email your teacher.

STEP 3

Make sure you see your own name on the survey. Then click, "Start Surveys."


STEP 4

Choose an answer for each question by clicking the circle under your answer choice.


STEP 5

Once you've completed all of the questions, click "Submit" at the bottom of the page.

You will not be able to change your answers once you click "Submit."

STEP 6

When you have finished taking the survey, you'll see "Thanks for taking our survey!" on the screen.
