To Submit Athletic Permission Forms Online

1. Have your parent log on to the Genesis Parent Access Account (Link here). 

2. Click on Forms 

3. Click on the Sports Sign up for your season (Fall / Winter / Spring)

If a new physical evaluation is needed please click the link below to print out the forms. Bring the Physical Evaluation Forms to your family doctor for the evaluation. This form must be submitted to the school nurse.
