Referral Guide

This resource is designed to assist clinicians when advocating for OT services at their sites by assisting clients, their families, caregivers, and other health professionals basic information about when and how to refer to OT services. Providing information/education on how to request referrals expands access to OT services. The below information is available as a tri-fold through Canva or as a PDF (also below).

• Explains how to make a referral to OT services through Infinity Rehabilitation

  • First, go to, or call the Infinity Rehab office on campus. Speak with the OT about your concerns or goals.

  • Second, once insurance is verified and you have a physician's order

  • Then, schedule your first appointment with OT.
