Release Of Liability, Waiver Of Claims And Indemnity Agreement
By registering, you are confirming that you have reviewed and agree to the Release of Liability, Waiver of Claims and Indemnity Agreement if you are over the age of majority. If you are under the age of 18 years of age, a parent or guardian must read through the Informed Consent and Assumption of Risk Agreement.
For those under the age of majority (under age 18), a parent or guardian must type their name to assert that they have read, understand and agree to the statements above.
Liability Waiver
I hereby agree to waive and to hold harmless the "Ottawa Curling Club "(the "Club"), its officers, employees and volunteers from any claim whatsoever arising from personal injury and/or property damage as a result of use of the "Club" equipment and the premises, except claims arising from the negligence of the "Club", its officers, employees and volunteers.
I further agree to be responsible for any damages incurred to the "Club" equipment and premises during our use and will be responsible for paying to repair any such damages. It is also understood and agreed that the "Ottawa Curling Club" will not be held responsible for property, personal or otherwise, left on the premises.
Rowan's Law
Under Rowan’s Law (Concussion Safety) 2018, the Ottawa Curling Club asks you to confirm that you have reviewed the Concussion Awareness Resources on this website (www.ontario.ca/page/rowans-law-concussion-awareness-resources). By registering, you confirm that you have reviewed and understand the resources. If you are under 18 years of age, a parent or guardian must sign a form acknowledging that they have reviewed the resources - please contact the Manager.
Privacy Policy
The Ottawa Curling Club is committed to meeting the privacy standards applicable to every member. The OCC privacy policy includes protecting the privacy of the personal information you have provided us on this "Application for Membership". The OCC Privacy Information Officer is the club manager.
A copy of the OCC's privacy policy is available at the club and on the website.
By continuing, I hereby agree to the collection and use of my personal information solely as provided for in the OCC privacy Policy, a copy of which has been made available to me.
Code of Conduct
This document sets forth the Ottawa Curling Club’s values and expected behaviours that guide the relationships and conduct of members, employees, contractors and visitors, with the goal of enhancing and maintaining respectful relations and good sportsmanship. It offers guidance on what is expected of all Club members, employees, contractors and visitors in their day-to-day interactions and enjoyment in relation to their Club experience.
