and which outside insitution you will attend to make up a D/F grade.
*Students must be signed into their OPUSD email account to access the form.*

Students interested in taking courses at Moorpark College should complete the following steps (see help guides below for more assistance):
B. Submit a digital MOU form
C. Register for the class
D. Complete the Outside Credits Form if you want the class added to the OPHS Transcript
*Use the form from #1 above if this college class is for first-time, acceleration credit
*Use the form from #2 above if this college class is for remediation credit to raise a low grade
NOTE: Students are welcome to take college courses from any accredited university/college. Please use information provided by the college of your choice to apply and register for the course.
