Application: FALL WINTER SPRING

Retreats must be paid in full at the time of application and payment turn in.

A deposit may be paid with prior approval if a payment plan is needed. However, all payment plans must be paid in full prior to retreat departure.

The April 10 - 13 retreat will be discounted for those in financial need. Contact Ms. Hazlewood by email regarding financial assistance.

Medical Form

Turn in a medical form only if you need to take medication on the retreat, including over the counter medication. You will turn in the medical form with the medication on the morning of the retreat. Please do not turn in the medical form prior to the morning of retreat.
