Please fill out the form below for your child to be able to participate in my class. All students are required to have Insurance to be able to participate in my class and must be updated each year in the program. They will also have a $35.00 lab fee each year for materials used in this program. IF you do not have insurance I will have a link at the bottom of this page to get school insurance so they can participate.
