Under what circumstances must I have a RTW form completed by my physician?

No. The Risk Department will review the form and make the decision on the employee returning to work. The Risk Department will then send the employee and his/her supervisor, timekeeper and the Leave Generalist in HR an email letting them know if the employee is approved to return to work or not.
What if my physician is on vacation when I need a form filled out?
A physician covering for your physician or a nurse on his/her staff can fill out the form if your physician is unavailable.
No, but any other form or letter MUST include the following:
1) the date he/she is approving your return to work
2) if there are no restrictions, or if there are, they are clearly stated AND the date those restrictions will end
3) If he/she doesn't know the date of the end of your restrictions he/she will enter the date of your next appointment.
