CHS Grad Party Event Agreements

     This event provides a safe place for Churchill High school graduates to celebrate with their classmates. The CHS graduate attending the event agrees to:

• Follow all rules and regulations set forth by the grad parent event committee as well as the staff at the venue site, including COVID-19 related regulations.

• Abide by the no alcohol, drugs, or tobacco possession or use policy, which includes vaping. Violations will result in immediate dismissal from the event and the venue. Your parent or legal guardian will be called to come pick you up. 

• Graduates will not be allowed to re-enter the event should they choose to leave. Must be present @ 2:30 am to win raffle gift baskets and grand prizes.

• Bring minimal items to the grad party. A bag will be provided for your belongings. All bags are subject to search at anytime during the event.

• Authorize chaperones at the event to obtain medical care for the graduate in case of an emergency.

• Assume full liability should the graduate require medical treatment.

                                         CHS Grad Party Waiver & Release

The grad party event is organized and managed by a parent committee. It is not school sponsored or associated in any way with Churchill High School or the 4J School District. The following terms apply:

• I certify that I am aware of no undisclosed health-related reasons or problems that preclude the graduate's safe participation in any of the activities at the event. I further understand that the graduate’s participation in this event is completely voluntary.

• I agree to release the members of the grad party parent committee, its agents, and volunteers from any and all liability, negligence, and expense in any way resulting from, related to, or arising out of the graduate’s attendance and participation in the CHS Grad Party on June 11, 2024, including but not limited to liability and expense attributable to Covid-19 illness, any injury, death, property damage, lost wages, economic loss, emotional distress, psychic injury, pain, or suffering of any kind whatsoever.

• I agree to hold harmless the grad party parent committee and all of its respective agents and volunteers from and against any and all claims, liability and expense in any way resulting from, related to, or arising out of the graduate’s participation in the CHS Grad Party event.

• This release shall be binding upon the graduate and their parent(s)/guardian(s), their respective heirs, and legal representatives. I understand that by signing this agreement I am releasing claims and giving up substantial rights, including my right to sue.
