Dear Parent/Teacher,

In working with your student in individual and small group counseling it is helpful to gather information from important people in the child’s life. It is helpful to understand your perception of how the child feels and thinks about him/herself, your perception about how he/she relates to others, and about how you think he/she is doing at school. Thank you for taking the time to complete this inventory and send it my way. Information will be used to better understand the child as we work together to help him/her be successful.
