Preschool students are required to complete a health information form and medical statements form. These forms allow us to safely care for your child during the school day.
Please note that the Child Medical Statement (Physical) is only good for 1 year from completion of the last exam. To maintain compliance, this will need to be updated accordingly. Thank you.
