Piedmont Free Physical Sign Up - April 30, 2022Date/Time: Saturday, April 30th at 8:00 AM Location: Piedmont Sports Medicine 1305 Jennings Mill Rd Building 300, Suite 110 Watkinsville, GA 30677 Student-athletes are responsible for their own transportation. Masks are required to enter the exam queue. Student-athletes must bring a copy of the GHSA physical form. Parent/guardian must complete and sign the health history form prior to check-in. No exceptions. Detailed information and blank copy of the GHSA physical examination form (four pages) will be sent to the listed email after registration. UPDATED PHYSICAL & DOCUMENTS DUE:

Physicals for athletes should be uploaded through Dragonfly. This also houses other important documents required for participating in a sport. It is the athlete and parents responsibility to have their physical uploaded into Dragonfly before they can participate in tryouts or the sport.

Rising 9th graders: Make sure to get a copy of your physical from the middle school OR your pediatricians office. Once you have the physical you can then upload it into Dragonfly. Don't forget to sign the other forms as well. Reach out to Coach Hinshaw if you have any questions!
