Every student athlete and their parent/guardian must complete Concussion Code of Conduct forms prior to the student athlete participating in interscholastic sports. This form has to be completed ONLY ONCE per school year (ie. 2023-2024 school year).

A student with a diagnosed concussion requires cognitive and physical rest followed by stages of cognitive and physical activity which are best accommodated in the home environment under the supervision of the parent/guardian in consultation with the medical doctor or nurse practitioner or other licensed healthcare provider.
A student with a diagnosed concussion requires cognitive and physical rest followed by stages of cognitive and physical activity which are best accommodated in the home environment under the supervision of the parent/guardian in consultation with the medical doctor or nurse practitioner or other licensed healthcare provider.
A student cannot return to school following a concussion diagnosis until parents/guardians complete the Home Concussion Management Form (link below) and return it to school where they confirm the student’s successful completion of Stage 2 of the Return to Learn plan.
