We are pleased to share that the South-East Ottawa Community Health Centre Primary Care Clinic is currently accepting new patients from select communities. 

At this time, our attachment opportunities are available for individuals who do not have a primary care provider and who reside within one of the following postal code areas:

For direct registration, Individuals and Families meeting this criteria are encouraged to complete the clinic's pre-screening form by scanning the QR code on the attached flyer. Upon submission, a member of the Primary Care Intake Team will review the information and contact eligible individuals within the following weeks for follow-up.
