Student Work Permit

Step One: You must already have a job, permission from a parent or legal guardian, and be in good academic standing to be able to request a work permit

Step Two: You must complete Form B1-1: Statement of Intent to Employ a Minor and Request for a Work Permit-Certificate of Age which can be found at the following website:

• The student will need to complete the “minor’s information” and “school information” sections

• A parent or legal guardian needs to complete the “to be filled in and signed by parent or legal guardian” section

• The employer needs to complete the “to be filled in and signed by employer” section

Once all these sections are complete, send the B1-1 form to Dean of Students,, who will issue the actual work permit. If you have any questions or need assistance,  please email the Dean.
