If a student needs to have medication administered during the school day, a medication administration form MUST be completed and signed by a parent/legal guardian AND a licensed prescriber. This includes prescription and over-the-counter/non-prescription medications, (even if the student is 18 years old.)
If a student needs an emergency medication such as an epi-pen, inhaler, or diabetic supplies, those medications may stay with the student at all times. All other medications will be stored in the clinic.
Please click on clinic forms to retrieve a copy of the self-carry and general medication administration forms. Remember: "new year, new form". A current completed form is needed for each school year respectively.
If a student does not have the required form(s) completed form on file, a parent/legal guardian may check into the main office to administer medication to their child.
END OF THE SCHOOL YEAR: Please make arrangements to pick-up your child's medication by the last day of school. ALL remaining medication will be discarded.
Please turn in updated immunization documentation directly to the clinic.
Documentation may include a copy of an updated immunization record or a signed statement from a parent/legal guardian or physician, (even if the student is 18 years old,) exempting student from receiving vaccine (s). Please click on clinic forms to locate an exemption letter.
At this time, immunization documentation may be sent via mail, email or fax with attention to the clinic.

A written note from the doctor, athletic trainer, or parent requesting use of the elevator is preferred. Elevator passes may be signed out and returned to the clinic. In the event that there is a fire drill or an actual fire, the elevator is NOT to be used. Should a student be on the second floor during a time that the elevator cannot be used, a student may have staff assistance. to descend the steps. There is an Evac chair located at the top of the steps at the end of the senior high wing. A student may have the assistance of one other student to assist with carrying belongings while using the elevator. Students are welcome to report any issues regarding the elevator to the nurse.
Immunizations: - ATTENTION incoming 7th and 12th graders for the 2025-2026 school year. Please consider scheduling for your required vaccines for the upcoming year beginning now through the summer months!
