Student Health Records

The school nurse and clinic staff are responsible for maintaining accurate student health records. This includes the enforcement of state laws regarding immunizations.

It is the parent/guardian’s responsibility to update their child’s medical records and documentation. 

Immunizations 

In Ohio, children must have been immunized against the common childhood diseases prior to entrance to school. If you have any questions about immunizations, please call the clinic or the Clark County Combined Health Department. Immunization records should be provided to the clinic on or before the first day of school. 

By state law, any student not up to date on immunizations must be excluded after 14 school days if he or she is not in process of obtaining these immunizations.

**Students entering into 7th and 12th grades require the following immunizations:

*7th Grade - first dose Tdap and Meningitis (MCV4)        

*12th Grade - second dose of Meningitis (MCV4)

Clinic Guidelines 

The school clinic is a first aid station for problems occurring at school. It is not a diagnostic clinic for illness or injury. Please do not send your child to school when he/she is obviously not feeling well or has symptoms of illness. That being said, our nurses and staff are always available for consultation regarding health issues that affect your child’s education.

If your child becomes ill or is injured while at school, and the illness or injury is medically significant or may become significant, the nurse or a member of the office staff will notify the child’s parent/guardian. If we are unable to reach either parent or guardian by phone, the alternate person (as designated on the Emergency Medical Authorization section of FinalForms) will be called. It is very important that parents provide the school with updated contact and medical information on the Emergency Medical Authorization Form available through FinalForms. 

Clinic visits and/or calls from the clinic for student pickup, do not waive missed hours of attendance. For questions or concerns regarding student attendance, please contact the main office. 

If you and your child have made previous arrangements and/or communicate during school hours via email/text/call regarding your child leaving or being picked up from school early, please communicate that through the main office. 

Sickness Policies

Students will be sent home if they are exhibiting the following symptoms:                         

• Fever (over 100)

• Vomiting for a reason possibly having to do with a contagious illness

• Active head lice

• A generalized rash of unknown origin

• Any other symptoms at the discretion of the nurse.

Parents are responsible for providing or arranging transportation for children who are sent home. If a fever has been confirmed, students must be fever-free without the use of medication for 24 hours before returning to school. If a student has had more than one episode of vomiting, there should be 24 hours since the last episode before the student returns to school.

Medication 

For students who require prescription medications during the school day, a completed medication authorization form signed by a parent/guardian and the prescribing healthcare provider must be submitted to the clinic. All medications must be provided in their original containers and are kept in the clinic.  When in FinalForms you will be prompted to print and complete this form, IF you select that your child requires prescription medication during school hours.

For students requiring over-the-counter medication, a medication authorization form must be completed by the student’s parent/guardian and on file with the clinic. All medications must be provided in their original containers and are kept in the clinic. When in FinalForms you will be prompted to print and complete this form, IF you have selected that over-the-counter medication may be given to your student.

Forms are only good for one school year. New year = new forms!

Medical Needs

Our student population includes students with a variety of special medical needs. The school nurse and support staff assist in meeting these needs as directed by the signed treating physician orders on file at the school. Please ensure if your child has special needs including but not limited to diabetes, seizures, allergies requiring rescue medication, asthma, etc. the proper documentation and supplies are provided to the clinic. 

Screening

In the interest of student health and in promoting good health practices, Northwestern Local Schools conducts both vision and hearing screenings. These screenings are conducted according to guidelines of the Ohio Department of Health. These screenings include hearing and vision screenings for grades 9 and 11, as well as all new students to the distinct, regardless of grade. Vision screenings only are conducted for 7th grade. 

If a student does not pass a screening, we will rescreen in a few weeks.  If the student still shows signs of vision or hearing difficulty, we will send a referral letter home, recommending evaluation by a vision or hearing specialist.  Keep in mind that these are screenings to detect possible problems and should not take the place of an exam by your doctor.

Elevator Passes

A written note from the doctor, athletic trainer, or parent requesting use of the elevator is preferred.  Elevator passes may be signed out and returned to the main office.  In the event that there is a fire drill or an actual fire, the elevator is NOT to be used.  Should a student be on the second floor during a time that the elevator cannot be used, a student may have staff assistance. to descend the steps.  There is an Evac chair located at the top of the steps at the end of the senior high wing.  A student may have the assistance of one other student to assist with carrying belongings while using the elevator.  

Upcoming Events

• Immunizations records are due no later than September 4, 2026 to prevent possible school exclusion    

• Hearing and Vision Screening Grades 7,9 and 11- September 30, 2026

• Mobile Dental Clinic - October 2, 2026

• Mobile Vision Clinic- January 2027
