Important Reminder:
All students are required to have the following updated immunizations by the 15th day after school begins.
Listed below are the Immunization requirements for Kindergarten:
DTaP: 4 or more doses (dose 5 required if the 4th dose given prior to the 4th birthday)
Polio: 3 or more doses (dose 4 required if the 3rd dose given prior to the 4th birthday)
MMR: 2 doses
Hepatitis B: 3 doses
Varicella (chickenpox): 2 doses
When you receive a vaccine at any time throughout the year please bring a copy of your shot record to the clinic!
Yearly Screenings:
Hearing and Vision screenings are completed during the first two quarters of the year for the following grades:
Pre-K, K, 1st, 3rd and 5th Grade
Referral letters will be sent out for students who do not pass the hearing or vision screenings.
Community Health Partners of Western Ohio
Currently, Northwestern Local Schools partners with CHP of Western Ohio to provide Dental and Vision exams. Consent forms are sent home at the beginning of the year, parents can select for students to participate in the Dental Exam Only, Dental and Vision Exam, or neither.
