Medical Form Update

Please update the medical update form ANNUALLY via the Parent Portal. Also contact your school nurse during the school year if there are any changes in your childs health due to:

Serious illness

Recent surgery or injury

Medication: prescription or over the counter

Chronic illness: life threatening allergies, asthma, diabetes, seizures, heart/lung problems, ADD/ADHD, mental health concerns, migraines, or digestive or elimination problems.

Our nursing staff is available daily to provide students with quality care during the day to promote healthy school attendance and participation.
