New or Returning Students (withdrawn from CCSD for 30 days or more).

• Student's original birth certificate (translated if not in English)

• Immunization Record
  Student's complete immunization record (if you're a NV resident, you can find them here).

• Parent/Guardian ID
  Must be a valid driver’s license or government ID that matches the parent on the student’s birth certificate.

• Proof of Residency
  Gas, water or electric bill (must be dated within 30 days).
  Either form of proof must match the parent/guardian ID.

Note: If you only have a residence lease or sales contract as proof of address, the school will register your child on the condition that one additional proof of address from the above list is submitted within 30 days. Driver's license, telephone bill or cable bill are NOT acceptable. 

• Withdrawal grades from prior school (outside of CCSD) and/or transcript.

• IEP/504 (if applicable)

All required documents must be submitted at the time of enrollment, partial submissions will not be accepted.

Current CCSD Transfer Students

• Proof of residency & Guardian ID.

• 12th graders are required to provide proof of 2nd MCV4 vaccine.
