The Clark County School District (CCSD) Regulation 5111.1 requires a parent/guardian to provide a proof of residency to enroll their child in school. In an effort to centralize the process for shared housing situations, the Education Services Division (ESD) outlines the necessary changes to replace the former residential affidavit process. The Title I Homeless Outreach Program for Education (HOPE) eligibility process will remain consistent through the onsite form collection.

Below is the list of all the documents required to process your child’s application:
A notarized copy of the Shared Housing Verification Request Form.
Government-issued photo identification with address showing parent/guardian is living at shared housing address (e.g., driver’s license, state issued identification card, passport).
AND
Two additional supporting documents in the name of the parent/guardian at the address shown on the Shared Housing Verification Request Form, dated within 90 days of the current date. The document should be communication to the parent/guardian at the address from the following type of entities:
Financial Institutions (i.e., bank, insurance company)
Utility Company (i.e., phone, gas)
Medical
Religious Institutions
Governmental (i.e., federal, state, local)
All required documentation should be submitted through the Online Registration Application. If an Online Registration Application has already been submitted and approved, the Alternate Shared Housing Application can be utilized to submit the Shared Housing Application.
The ESD staff will notify families of the outcome via email.
Yes. Appeals may be submitted to ESD via email at 151-SharedHousing@nv.ccsd.net.
Secondary students attending on Shared Housing are ineligible for athletic participation. Eligibility may occur through Nevada Interscholastic Activities Association (NIAA) hardship. Please contact your school's Athletic Office for more information.
Verification of residency may include visits by attendance officers. This process may occur on an ongoing basis to verify residency aligned to the approved Shared Housing Verification Request Form.

Yes. The Shared Housing Verification Request Form must be completed each year.
