Registration

Now that your scholar has been accepted please begin by printing and signing the following registration forms:

• Scholar Acceptance Notice

• Scholar Registration + Emergency Information 2022-2023

• Home Language Survey

• Release of Records

Registration Documents

After you've completed the online registration, please stop by to submit the following documents:

1. Original Birth certificate

2. Two proofs of address (Utility Bills, lease, bank statement, or certified mail)

3. Physical Exam

4. Dental Exam

5. Pay $100 NON-REFUNDABLE annual fee

6. Vision Exam (ONLY for scholars entering Kindergarten or transferring scholars from another state)

7. CPS Family Income Information Form 2022–2023

Annual Fees

There are two ways to pay for your scholar's $100 annual fee: Make the full payment or start a payment plan.

1. Full Payment

Stop by the main office and one of our clerks will be able to take a cash or card payment.

2. Payment Plan:

Stop by or call the main office and one of our clerks will be able to help start a payment plan that works best for you.

Once our scholar's annual fee is paid, you will receive one PE uniform with your payment.
