As both a nurse and nurse psychotherapist, I fulfill this requirement.
In order to evaluate your need for a service animal (SA), I need to perform an assessment to understand the areas of your life where a service animal can perform a function.
As part of my practice, I also work with a dog trainer, who also has a service animal, to help me to ensure you understand the time, commitment and responsibilities inherent in having these special animals in your life and taking them in public to support your disability.
If you are qualified, after this assessment, I will write you a letter substantiating your need for a service animal. This letter is only valid under the following circumstances:
Yearly - with your attestation to no changes in health and to the health of your service animal.
6 Months - if you have a Service Animal in Training (SAIT), and then follow-up every 6 months until your SAIT is fully grown and operating at full capacity. Once your SA is at capacity and fully functioning, and your disability is being adequately supported, follow-up is yearly.
As a member of a regulated health profession, I am able to work collaboratively with your current health team. Either you or they can refer you to me. They can fax a referral to me at: 647-558-4579

Initial Assessment of Disability-Related Need and Use of Service Animal - $450.00 (approximately 1-2 hours) In-Person Assessment in Mono, Ontario (includes assessment by service animal dog trainer).
Qualification Letter - $150.00 (see above for letter expiry).
*This cost qualifies as a medical expense, and you can claim this on your taxes. An invoice with a detailed cost breakdown and Regulatory College # is provided.
*If our location is far from you - barrier-free accommodation can be arranged (at an additional cost). In some circumstances, we can come to you.
