In Ontario, People who need support from a service animal are required to have a letter supporting this need from a regulated health care professional from this list: (www.ontario.ca/page/accessibility-ontario-what-you-need-to-know)

• audiologist or speech-language pathologist

• chiropractor

• nurse

• occupational therapist

• optometrist

• physician or surgeon

• physiotherapist

• psychologist

• psychotherapist or mental health therapist

In order to evaluate your need for a service animal (SA), I need to perform an assessment to understand the areas of your life where a service animal can perform a function.

As part of my practice, I also work with a dog trainer, who also has a service animal, to help me to ensure you understand the time, commitment and responsibilities inherent in having these special animals in your life and taking them in public to support your disability.

If you are qualified, after this assessment, I will write you a letter substantiating your need for a service animal. This letter is only valid under the following circumstances:

• Yearly - with your attestation to no changes in health and to the health of your service animal.

• 6 Months - if you have a Service Animal in Training (SAIT), and then follow-up every 6 months until your SAIT is fully grown and operating at full capacity. Once your SA is at capacity and fully functioning, and your disability is being adequately supported, follow-up is yearly.
