Records Request: Alumni and Former Students

Do you need a copy of your high school transcript, immunizations, or other student records? We can help and there is no fee for this service.


Click on the link below and complete the Records Request Form in it's entirety. Email your completed form to Candace Hunt at chunt@nu-district.org. For verification purposes, you must also include a copy of a photo ID.


Please understand that documents are only considered "official" if they are sent from North Union Local Schools directly to a college, university, technical school, or other institution. During the school year, we ask that you allow 48 - 72 hours for processing. Questions, please call Candace Hunt at 740-943-1851.


Our staff takes time off during the summer months. You may submit record requests during this time but please realize staff may not be working at the time of your request. All summer inquiries will be completed in early August upon staff return.
