Health Examination Requirements

New for 2018 it is required by New York State that students New Entrants, Kindergarten, 1st, 3rd, 5th, 7th 9th 11th grades receive a Health Examination by a NYS licensed provider. This examination must be provided to the Health Office within 30 days of entry into school. If your child has a physical scheduled during the school year, please notify the health office of the date of the scheduled physical exam.When no documentation is received by the school district, an examination will be conducted by a BOE appointed school physician during that school year at your child's school.

Dental Certificate

Dental Certificate is requested, not required, at any time a health examination is required, so Kindergarten, 2nd, 4th, 7th and 10 grades.
