Physical Examinations and Immunizations: Massachusetts State Law mandates written documentation of up-to-date immunizations before entrance into the Northborough-Southborough Public School district. A copy of a recent physical exam is required before students enter kindergarten, fourth grade, seventh grade and tenth grade, and for all new students.Â
